Paper: 20lb NonGlossy White, 2 sided!

Signature: Signature as it appears on your driver's license.

Printed Name: Print the name that appears on your driver's license.

Date Format:
All signers must write the date in this format: MM/DD/YY, using numerical values only.-
Example: Use 08/06/25, not "Aug 25" or "August 25".

For single-digit months or days, always include a leading zero. (e.g., 02/05, not 2/5)

Address:
Street Address on Driver's License, City, and Zip Code. State is not necessary. Corrections/additions must be initialed. -
Certification Section (Bottom of Petition):
You, as the circulator, must WITNESS the signatures and sign and date the certification at the bottom of each completed petition sheet. DOES NOT REQUIRE A NOTARY! If you are signing a single petition, your signature is the certification.-
The date you write must be the same as, or later than, the last date written on the signature lines.

Do NOT use a date earlier than the last signature date—this will result in the entire sheet being rejected.
